Child U.S. Passport Application
Applicants under 16, Form DS-11

You must first meet with an Acceptance Agent before shipping us your passport application for processing.

After you have completed the DS-11 passport application and have collected all of the supporting documents, both parents and minor must appear in front of an Acceptance Agent to have your passport application executed. The Acceptance Agent will review your documents, ask you to sign the application, they will seal it in an envelope, and hand it back to you. You will ship us this sealed envelope.

Once you have met with the Acceptance Agent, ship all of your required documents to us immediately.  We must receive your documents within 4 business days of meeting with the Acceptance Agent.

Note: Many Acceptance Agents may not accept your documents unless they see your departure date is within 14 days. It is recommended to contact them first to check if they enforce this rule.

• Verifies your identity
• Verifies your documents are in order and collects the $35 Execution Fee
• Administers an oath in which you will swear that you are submitting true and correct information in support of your passport application. You will sign the application in front of the Acceptance Agent.

• One (1) unsigned DS-11
• One (1) unsigned Letter of Authorization
• Original proof of citizenship
• Parents' proof of identity
• Statement of Consent: Issuance of a Passport to a Minor DS-3053 (If applicable)
• Proof of travel
• One (1) passport photo
• Check or money order payable to "U.S. Department of State"

• Print two (2) copies of the (completed) passport application.
  • One will be presented to the Acceptance Agent to be used in processing your passport, and the second will serve as reference for us should there be questions regarding your application. 
• Do not sign the application until you are in front of the Acceptance Agent. (further details in Requirement 1 - Acceptance Agent Process)
• The application must be Completed Online - handwritten applications are not accepted.

Be sure to enter your child's social security number (SSN) on the passport application. If your child does not have a SSN you will need to submit a signed letter of explanation stating one of the following:

• The child has registered for a SSN and you are waiting to receive it
• The child has not yet registered for a SSN but will do so as soon as possible

Please print and complete two (2) copies of the Letter of Authorization

Important:

• All signature areas must be signed with black/blue pen. Electronic signatures will not be accepted.
• The letter must be printed on 8 1/2 x 11 size paper. No other size paper is accepted. 
• Passport applications for Minors, must enter child's name as applicant's name.

The DS-64 Statement must be completed if the most recent passport cannot be submitted.

Complete and sign the Statement Regarding a Lost or Stolen Passport DS-64

Your Certified Birth Certificate issued by state, county or city (if born in US) is required. Note: Applicants born in CA and TX must use the Long Form birth certificate. Birth abstracts are not accepted as proof of citizenship.

In addition please provide ONE of the following if applicable:
 

• Previous U.S. Passport- If you have a previously issued US passport it must be submitted. If you cannot locate the passport you must apply for a lost passport.

• Certificate of Citizenship or FS-240, DS-1350 (if born outside the US)

• Certificate of Naturalization

• Please thoroughly review the Parental Application Permission Documentation
• In addition parents will need to submit a form of ID. The best form of ID is a valid driver license. The license must have been issued a minimum of 4 months ago and display your photograph.

If one of the parents will not be present when applying please complete the Statement of Consent Issuance of a Passport to a Minor . The signature must be witnessed and notarized.

• Statement of Consent- to be completed by the non-applying parent or guardian when he or she will not be present at the time the applying parent or guardian submits the minor's application.
• Statement of Special Circumstances- to be completed by applying parent or guardian when the written consent of the non-applying parent or guardian cannot be obtained.

Provide a copy of one of the following. (Applicant's name must appear on documentation):

• Copy of a plane ticket to a foreign country or;
• Copy of travel itinerary indicating dates of international travel or;
• Business travelers may submit a [DOCUMENT!]

Submit one (1) recent passport photograph taken within the last 6 months.
Eyeglasses cannot be worn in your passport photo.

You will write a check to the "US Department of State" for $186.87 (please write your date of birth on the memo line of the check).

Passport Cards- If you are also applying for a passport card you must send a check in the amount of $201.87 payable to the "U.S. Department of State". Passport cards cannot be expedited and will be mailed to the address indicated on your application.

IMPORTANT NOTICE REGARDING FEES!
The Acceptance Agent may ask you to write a check for a different amount than we have instructed. If so, please contact us immediately to let us know.  This is very important as it allows us to ensure the timely processing of your passport application to meet your travel needs.

Starter checks (checks without a pre-printed address) and 3rd party checks are not accepted.

After you have met with the Acceptance Agent, hand fill out the Passport Application Shipment Checklist and include this in your shipping envelope.
